Needs Assessment Service Coordination (NASC) provides comprehensive needs assessment and service coordination to enable people of all ages who have either a disability or age-related condition to live safely and independently in the community for as long as they can.
The Older Persons service is located at Napier Health.

Referrals for basic housework support are usually done by your healthcare provider or from hospital staff before you are discharged.
