New Zealand Smoking Monitor 2011/12. Questionnaire.

The HSC runs a continuous tobacco control survey (New Zealand Smoking Monitor - NZSM) on behalf of the Ministry of Health to measure current and changing knowledge, attitudes and behaviours relating to smoking and quitting. It offers a unique way to monitor trends over an extended period of time
