Services we provide
Te Ara Tiaki Pakeke provides support for older people through services and teams including:
- home and community support services — home help or personal care for short-term support or after a major change in health
- clinical nurse specialist who works alongside the geriatrician and the wider team to do geriatric assessments and work with stroke survivors
- working with people aged 65 and older, or under 65 with an age-related condition or living with a long term or chronic health condition that is not a disability
- mental health needs assessments and services coordination
- fracture liaison service — a service which screens patients age 50 and over who fall and break a bone that is not in the hands, feet or skull
- work as part of the allied health response service, providing assessment of frail older people to ensure safety, and to help with their independence at home after a visit to the emergency department or a serious decline
