You will be referred to occupational therapy services if you:
- have a decline in functional ability due to hospital admission or a recent event
- show cognitive impairment that affects your ability to function or be safe at home
- live alone with limited social support
- need checks or home visits to assess safety
- require equipment on discharge and need long term funding for it.

You can be referred to occupational therapy services by your healthcare provider or another health professional involved in your care.
